It is a challenge before cancer surgeons to renew the breast
which is being removed. Researches are going on regarding this issue in the whole
world and it is possible that such surgery maybe an option in the coming
decade. Young woman with small breasts face problems and side effects in such
kind of surgeries.

This information was shared by oncologist Dr. Sarika Lamba
from Narayana hospital. Dr. Sarika was addressing a meet organized by SMS
hospital, Sita Devi Plastic surgery department and Rajasthan Breast oncology
group. She shared that the treatment of cancer depends on the stage of the
patient. If the patient comes on time then this treatment is successful. If the
cancer is found in its first stage then there is only 0.5 percent chance of it
occurring again. If the cancer is at third stage then 20 percent of patients
have it recurring again. Dr. Uttam Soni shared that stem cell cannot be used at
present in cancer treatment.
The researches regarding stem cell is going on in the whole
world. The key is to find out which stem cells help to cure cancer. At present
only few stem cells have been discovered and used for treatment. But still there
is no success in case of cancer. Also it is not clear as to which therapy is to
be given to patient in few cases.

MCI Notification- Doctors would have to write medicines in capital letters
Prescribe and write medicines in capital letters only : MCI
To help the patients avail cheaper medicines rather than branded medicine the doctors would have to write the medicine generic name in English in capitals from now onwards. This notification was issued by the medical council of India New Delhi last month. The Central government has made amendments to Indian medical council regulations act as per 2016. The doctors would have to write the generic name of medicines in capitals for example if someone has fever then the doctor has to write paracetamol instead of Cronin.
Differences between generic and branded
Generic medicine is cheaper as it is not associated with any brand. In 2002 MCI had appealed doctors to write generic names of medicines but this became a big fight at that time. According to Indian journal of medical ethics 2007 report companies make two kinds of medicines ie generic and branded. But both have similar composition, quality, and performance. There is no difference in the process of making them also. The companies don't spend on any marketing or branding of generic medicines and sell them at a little profit. For writing branded medicines the companies give doctors gifts and commissions. This creates a price hike. Though national drug price control authority keeps a tab on prices from time to time.
To give respite to patients the government has decided to open 3000 Jan aushadhi Kendra in the first phase. These centers will only sell generic medicines at low cost. At present, the share of branded medicines in the market is 75 thousand crore rupees whereas generic medicines have a market of 8 thousand crore rupees. The chemist will have the power to give generic medicines to patients even if they have brought a branded prescription. A software is being created for the same wherein as soon as the branded medicine name is put it will show its equivalent generic medicine name and price.

Research on Dengue patients conducted by doctors at Kota Medical college
Research on Dengue patients under Dr. S.N Gautam of neurosurgery department
Now the whole world will learn about the research on Dengue patients conducted by doctors at Kota Medical college. This research will serve as a guideline in those cases where brain hemorrhage or hemorrhage fever happens due to Dengue.
The research has been accepted for the international journal. It will be published in its next issue. The research was based on the theme that which Dengue patients can benefit from surgery and which can't. The result was that if the hemorrhage is deep inside the brain then there is no point in surgery and is very risky. Under Dr. S.N Gautam of neurosurgery department, this research was done by the doctors on 1600 dengue patients at the medical college. This is one of the first research in neurosurgery in the case of Dengue.

Around 1600 patients of Dengue visited the Kota Medical college between Jan and Dec 2015. Around 300 patients were admitted due to bleeding and other reasons.
About 18 patients were diagnosed with Hemorrhagic fever and were admitted in neurosurgery. Last year Hemorrhagic patients in Dengue was 1.2 percent which is high and out of which 6 cases were operated and one died.
Those patients who were saved by operation had the extradural or subdural hematoma. But the patient who died had deep hemorrhage which is known as the intracerebral hematoma.
The research made it clear that while undergoing operation it has to be noted that the patient has been admitted on time, is less than 60 years and doesn't have other diseases like diabetes and asthma and their platelets should be at least 1 lakh.
Operation process
The first layer in the brain is bone, the second layer is dura and the third layer is the brain itself. The hematoma between fist and the second layer is called extradural hematoma, between the second and third layer, is called subdural hematoma and inside the brain is called intracerebral hematoma.
The research has made it clear that intracerebral hematoma should not be operated. If the first layer has hemorrhage then neurosurgeons open the bone layer clean the bleeding and hemorrhage and then reseal the bone layer. If this hemorrhage is in the second layer then bone and dura also have to be opened. If it is inside the brain then after two layers the brain also has to be opened.
Why does hemorrhage occur?
When platelets become very low then blood clotting process becomes weak and bleeding starts and hemorrhage occurs. Platelets help in blood clotting.
Mahavir Cancer Hospital on defaulting the land allocation rules

Ramesh Meena of Congress Dal has put allegations on Mahavir Cancer Hospital on defaulting the land allocation rules time and again. He has requested to cancel the land allocation of the hospital from the state government. Ramesh Meena addressed to the media and said that 22166 square yards land was allocated to trust on the token money of Re 1.
The rule to be followed by the trust was that free checkup and cure has to be provided to 25% poor and all BPL card holders. But these rules are not being followed. Flaws and loopholes have been found in committee selected by State government for the same.He warned that if the hospital administration doesn't follow properly then not only this issue will be raised in Vidansabha but also a protest would be carried out.
Maj General Dr. S.C Pareek of Cancer Hospital said that the hospital is being run on trust's main motive no profit no loss basis. They have followed all the rules which were decided at the time of land allocation. He added that even after working for the betterment of society some people want to defame the hospital.
No free treating poor patients by Jaipur Hospital after taking land from Rajasthan Government

A new case of hospital land being taken on nominal rates and not giving patients free cure has come forward. The hospital administration even sold off the land taken on nominal rates to other people. This complaint was registered at Lokayukta office. The person who bought the hospital also registered a FIR that no information of the hospital being under Trust was given to him before the sale deed. According to the case, nominal rate land was taken to open RG hospital at Mansarovar.
Later the trustee handed over the hospital to four people of a family. Mahesh Sharma the trustee contacted Somendra Ghosh on lieu of not being able to handle the hospital operations. Somendra Ghosh registered a FIR that 20 lakhs were taken on the sale of the hospital stating that the hospital can be run for 10 years. Some days later Mahesh Sharma did an agreement stating that 80 percent collections would be of the hospital and 20 percent collection would be of trust.
Lokayukta looked into this matter. The hospital was given 8750 square meter land at a nominal rate of 460 Rs. per square meter. For many years no construction was done on it. RG hospital's land has been taken on nominal rates but no free cure facility is given to patients. Also, thousands of rupees have to be spent on simple diseases. If proper action is taken then the people can get the proper cure at nominal rates here.

A new technique of endoscopy called Parallel endoscopy myotomy along with spy scope would be available in Jaipur now. Live operations would be performed on Sunday through this technique. This information was shared at the conference of Society of Gastrointestinal endoscopy of India at Rajasthan branch. The organizers shared that through this technique those patients would also be cured who have problems opening their mouth.
According to organizing secretary, Dr. Mukesh Kalla telescope facility is used to clear the blockage in the oesophagus in Poem technique. In this disease, the patient cannot swallow the food properly. Through Poem technique telescope is used to clear the blockage instead of opening the mouth. Spy scope technique is also being used for the first time in Jaipur. This is used to break the stone near gall bladder. Usually, this operation is done by opening the stomach. Jaundice is a symptom of this disease. Also, information would be given on using telescope technique to test stomach and liver ailments through the nostril.
NBI will detect the origin of cancer in the body-
NBI can be used to detect every part of the body without a biopsy. also, information would be given on endoscopy and Ultrathin transnasal endoscopy. The surgeries would be performed by Padmshree receiver Dr. Amit from Delhi, Dr. Sood, Dr. Mukesh Kalla and Dr. Monish from Chandigarh. These techniques will have fewer problems less cost and cure without operation.

Whenever free operation would be done at Bhagwan Mahavir Cancer hospital, its detail would be provided to SMS hospital administration. Also, free treatment of BPL patients who are referred from SMS hospital would be done. Even medicine amount would not be taken from the patients.
A meeting was held between SMS hospital administration and trustees at Bhagwan Mahavir cancer hospital where the above decision was taken. The head of Radiology, Dr. Arun Chowgle shared that the information regarding treatment and medicines are given to BPL patients was not coming through proper channel earlier. The trust officials have said about giving all the required infirmation in this meeting. Now it will be easier for BPL patients to get treated at the hospital.

Performance Review
The life-saving drug scheme is helping 2.5 lakh patients daily in Rajasthan. The budget has also been increased for the same. Around 206.66 crores were spent on medicines in 2012-2013 whereas 387.75 crores were spent in 2015-2016. The previous government had spent 537.31 crores in 26 months whereas present government has spent 943 crores in 32 months on medicines.Around 29.47 crores on average is being spent each month on providing medicines.Budget per year has increased from 200 to 400 crores. Around 200 crores of medicines have been purchased in the previous months of 2016-2017.
Cons - Not all medical shops have appointed pharmacists and the shops are open for few hours on holidays. Patients suffer from the unavailability of medicines. Medicines are kept in drug warehouse but no one is responsible for bringing them on time.
30 medicines have been added - Diabetes, antibiotics, and epilepsy medicines have been included. There are 17 medicines, 5 injectibles, 3 ointments and 4 surgical. The following medicines have been included in the associate drug list - Clobazam, Naproxen, quetiapine, lev teras ITM, methylcobalamin,vitamin d 3 oral solutions, gabapentin, sodium valproate, montelukast, tenalegalipetin etc. Injectables include lev teras ITM, insulin glargine etc. Insulin is being used for the first time.
Around 11 crores 61 lakh free tests
All medical colleges and hospitals associated with free tests scheme have done around 11 crores 61 lakh free tests till now. Medical college has 60, Jila and satellite hospital have 56, CHC has 37 and phc has 15 kinds of free tests available.
According to Medical minister Rajendra Rathore, the condition has become better. Allergy, Diabetes, Epilepsy,antibiotics and mental disease medicines have been included in the list. Around 10 crores are being spent each month more than the previous government.

The Rajasthan government has provided a clause to increase the financial limit in the medical diary of aged above 75 years of age but to avail this the pensioners have to present themselves in before zila committee with an application.
The pensioners have been asking to finish the limit of finance in the medical diary. Recently this proposal has been sent from pension department to finance department.
The finance department has sent this to financial rules department for its perusal. The government has provided a limit of 20000 in the medical diary of pensioners. This limit is renewed each year but if a pensioner requires more than 20000 in a year to spend on medical expenses then they have to present themselves with an application before zila committee.

Metro will
get the benefit of the land of Polyclinic Veterinary hospital land at Panch
Batti, M.I Road. Out of the eight thousand square meters land, around one
thousand will remain with the Veterinary hospital only. A hospital for small
animals would be functional here. The rest of the land would be given to metro
or will be auctioned and the budget would be given to metro.
On the other
side, JDA will provide five thousand
square meters land one four different
locations each to animal and husbandry department. This land would be allotted
in the forthcoming meeting of JDA. This decision was taken at the recent meeting in the presence of main secretary
Mr. C.S Rajan. However, there is no planning of metro on MI Road at present.

cheap quality of medical devices and their side effects along with the
medicines would be monitored in the treatment of serious diseases. The
government and the private doctors would have to send a form with the complete
detailed information regarding the effects and side effects of a cardiac stent,
heart valve, catheters, and other medical
devices. The team of specialists would not only alert regarding the quality of
the medical devices but also, the correct action would be taken against the
manufacturer under the drug controller journal of India, New Delhi.

decision was taken by the central government after adding sterol and nonsterol to the medicines. According to Dr.
Lokendra Sharma, pharmacology department, SMS medical college, Adverse Event
reporting form has been created by the central government to regulate and
inform the side effects of medical devices. In this form, the name of the manufacturer, category, license number,
brand, model number, serial batch number, lot number, the date of the implant on the patient and the side effects
would have to be mentioned.

Now the reasons for recurrent abortions in IVF treatment can be easily determined. There will also be ease in determining if children can get thalassaemia from parents.
All this is possible through the introduction of a new molecular lab in the state. This first of its kind molecular lab of the state was inaugurated at the Mahatma Gandhi hospital. Before the development of this lab, all samples for molecular genetics diagnosis were sent abroad. After the introduction of the molecular genetics machine at Mahatma Gandhi hospital, this is no longer required as it will be possible to get the diagnosis done here itself in the state.
According to a genetic specialist, Dr. Anil Dubey, “the preimplantation genetic diagnosis process will be very helpful to study the infertility gene in the molecular lab.” According to the hospitals chairperson, Dr. ML Swarankar, “this is the first of its kind machine in any of the medical college in the state.”

Tetralogy of Fallot (TOF) Surgery in Fortis Hospital Jaipur
Tetralogy of Fallot (TOF) operation performed of a 44 days old child.
A 44 days old newborn has been diagnosed with a rare disease. The impure and pure blood of the child was being mixed and was flowing in the arteries. The whole body of the child was getting blue and was in delicate situation. Dr. Sushil Kaushal of Fortis hospital performed Tetralogy of Fallot (TOF) successfully on the child.
According to Dr. Kaushal, the disease is controlled by BT Shunt Procedure and the surgery is performed after two to three years. But it is a life threatening situations for the patient in these two to three years. The VSD of the child was closed in such a way in the surgery that the pure blood from the left ventricle was joined with aorta with a tunnel-like tube. When the blood flow became normal then the path of blood flow was corrected from the right ventricle from the side of the lungs.
Why it was a challenge -
The child of Virendra Singh Rathore and Renu Rathore from Bhilwara was just 44 days old. The arteries, heart, and ventricular muscles were very small for the doctors to operate. Also, the small age of child was another factor in bearing the complicated operation. Dr. Rajeev Lochan Tiwari gave the anaesthesia. According to Pratim Tamboli of Fortis hospital, this is the first surgery of TOF at such a small age not only in the state but also in the country.

JK Loan hospital - No blood samples would be required for the testing of Jaundice in children.
With the onset of new machines at JK Loan hospital, blood samples would not be required to test the infection of Jaundice in children. It would be tested by a new machine Bera. The machine would be put on the ear and test sample, will be taken. Two other new machines have also been installed at the hospital. One of which would be used to test the condition of brain damage in the child.

Dr. U.S Agarwal of SMS medical college and Dr. Ashok Gupta of J.K Loan hospital inaugurated the three new machines namely – AEG, OAE, and BERA on Friday. According to Dr. Agarwal, “This is the first hospital in state where multispecialty machines super specialty cure have been used to test and treat children. This technology will also help in testing diseases in newborn.”
Monitor machine to test brain damage.
The condition of brain from an hour after birth to a month old would be tested through monitor machine. Until now, there was no facility to test brain condition until six months of age and this caused mental disorders in new born. According to Dr. Ashok Gupta, “The development of brain is completed in first 2 years of life of any human being.”
